Your medical forms

We do not use any of your data for marketing and will not contact you unless we need to discuss your case or appointment/booking. We may send you a Christmas card, just let us know if you would not like to receive one.

All medical data is stored inline with GDPR (2018) and medical confidentiality. Records are kept for 7 years from your last treatment, after which time they are securely shredded.  You can request a copy of your notes at any time within that 7 year period.
